Plantar Fasciitis — The Most Common Cause
Inflammation of the plantar fascia where it attaches to the heel bone. The classic symptom is severe pain with the first steps in the morning — then improving with movement, only to worsen again after prolonged standing or at the end of the day.
Who Gets It?
- People who stand for long hours at work
- Athletes, particularly runners who increase training rapidly
- Overweight individuals
- People with flat feet or high arches
- Those who wear unsupportive footwear
Treatment
- Calf stretching and plantar fascia stretching exercises — most important treatment
- Supportive footwear with good arch support
- Anti-inflammatory medications
- Corticosteroid injection for resistant cases
- PRP injection — excellent evidence for chronic plantar fasciitis
Heel Spur vs Plantar Fasciitis
Many patients are told they have a heel spur on X-ray and assume this is causing their pain. In reality, 15-25% of the population has heel spurs with NO symptoms at all. Treating the plantar fasciitis (not the spur) is the right approach.
Achilles Tendinopathy
Pain and stiffness at the back of the heel, particularly with the first steps after rest. Managed with eccentric calf exercises (the gold standard treatment), load management, and PRP injection for resistant cases.
